Permission for the Release of Information.  We need your written consent before releasing any information to a third party. This includes any written or verbal communication, such as a letter to your doctor or employer.

Consent for the Treatment of a Minor.  If you are bringing in a minor in for counseling or other services we need the legal parent or guardian to complete this permission form.
